"Here lies a perfect and upright man
the scholar, our teacher
Mordechai Zwi
son of R. Jesse
Jonasawicz (Jonasowicz) Jonasewicz Jonasowitz
He died 24th Adar 5685.
May his soul be bound in the bond of everlasting life."

The Aleph can be either an /a/ or /o/, so both readings are possible -
but surely one must be more accurate Polish name.
